Montgomery County Health Department/Home Health Agency provides in-home services to patients who qualify for skilled services.
To qualify:

A patient must have insurance coverage whether Medicare, Medicaid, or private insurance |
|
A patient must be homebound – he/she must have a physical inability to leave home. |
|
A physician must order medical skills, treatment, assessment or instruction in the home for the patient. |
Skilled services are provided by a:

Nurse |
|
Physical Therapist |
|
Occupational Therapist |
|
Speech Pathologist |
Home Health Aide services are available for personal care for those patients that qualify.
Home Health Care is for acute phases of illness and is not intended for long-term care.
Examples of “Skilled Service” could include:

Dressing change and wound assessment |
|
Cardiac/pulmonary assessment |
|
Diet instruction |
|
Post-op pain management |
|
PICC Line medication and management |
|
Rehabilitation services for speech, physical, or occupational therapy |
|
Catheter care |
|
Instruction to patient and/or caregiver on any of the above |
