Montgomery County Health Department is able to provide certified copies of birth and death certificates at their office.  You must provide proof of identity with a photo ID.  If ordering by mail, must send copy of photo ID.

Birth Certificates

Birth certificates are available for births in the State of Missouri from 1920 to the present time.  Cost is $15.00 per copy.  Must provide proof of identity.  Births from 1910 through 1919 must be obtained through the Vital Records office in Jefferson City, MO - call 573-751-6387.

Death Certificates

Death certificates are available for deaths occurring in the State of Missouri from 1980 to the present.  The cost is $13.00 for the first copy and $10.00 for every copy thereafter of the same certificate.  Must provide proof of identity.  Deaths before 1980 must be obtained through the Vital Records office in Jefferson City, MO - call 573-751-6387.
